I hereby voluntarily enroll in an exercise class conducted by Reising Wellness (“Reising Wellness”) and Pelvio Physical Therapy ("Pelvio PT"). I understand that participation in any exercise program involves risk of injury. I assume full responsibility for any and all injuries or damages that may occur as a result of my participation. I agree to follow all instructions and guidelines provided by Reising Wellness & Pelvio PT instructors. I understand that it is my responsibility to inform the instructor of any physical limitations, injuries, or medical conditions that may affect my ability to participate safely. I hereby release, waive, and discharge Reising Wellness, Pelvio PT, their employees, contractors, and agents from any and all claims, demands, actions, or causes of action whatsoever, arising out of or in any way connected with my participation in the exercise class.
