I hereby voluntarily enroll in a breath work class conducted by Rey Somatics (“Rey Somatics”) and Pelvio Physical Therapy ("Pelvio PT"). I understand that participation in any breath work program involves risk. I assume full responsibility for any and all injuries or damages that may occur as a result of my participation. I agree to follow all instructions and guidelines provided by Rey Somatics & Pelvio PT instructors. I understand that it is my responsibility to inform the instructor of any physical limitations, injuries, or medical conditions that may affect my ability to participate safely. I hereby release, waive, and discharge Rey Somatics, Pelvio PT, their employees, contractors, and agents from any and all claims, demands, actions, or causes of action whatsoever, arising out of or in any way connected with my participation in the exercise class.

The following conditions are contra-indications to breath work. Participants with the following conditions will, unfortunately, not be allowed to participate: 

Pregnancy (esp. first trimester)

History of seizures or aneurisms

Recent surgery

Severe asthma

Diabetes

High/abnormal blood pressure

Schizophrenia or bipolar disorder

Osteoporosis

Glaucoma
